"tlc" Wig Coupon Program

The American Cancer Society “tlc” Tender Loving Care® Wig Coupon Program helps cancer patients who otherwise would not be able to purchase a wig or halo.

Coupon Terms and Conditions

• The coupon is good for one wig or halo up to the value of $70. The coupon may not be redeemed for other “tlc” items, such as bangs, hats, or scarves. The value includes shipping and taxes.
• The coupon is good for one use only.
• The patient will be responsible for any costs above the value of the coupon, using either a debit or credit card.
• To redeem the coupon, the order must be placed by calling “tlc” Customer Service at 1-800-850-9445. The patient must reference the coupon code number during the call.
• The coupon will expire 30 days after it is issued. The expiration date will be noted on each coupon.
• A patient may redeem one coupon every 12 months.
• Coupons are only available in English and Spanish.

Eligibility Requirements

1. The patient must be in active cancer-related treatment (i.e., chemotherapy, radiation therapy, hormone therapy, or other forms of treatment) or up to six months post-treatment.
2. Patients are eligible regardless of insurance status. However, if the patient does have insurance, encourage them to check with their insurance company to see if they can provide additional assistance.
3. The patient has not received another wig or halo or “tlc” wig coupon from the American Cancer Society (regardless of source) within the past 12 months.

How to Request a Wig Coupon
If the patient meets the eligibility requirements, complete a Patient Referral Form provided to you by your American Cancer Society staff contact, and in addition to the standard patient information, include the following:

• In the comments section, note “Wig Coupon” and whether they want to have the coupon mailed or emailed to them.
• Capture mailing address and email address as appropriate.
• Capture language so the appropriate coupon can be provided.
• Fax the completed form to 1-877-428-2862 or email it to ssbcref@cancer.org.
